[NEW CLASS] By 2025 about 7 million Americans are expected to be living with dementia, and, if current demographic and health trends continue, more than 9 million could have dementia by 2030. The Guiding an Improved Dementia Experience (GUIDE) Model focuses on comprehensive, coordinated dementia care. It aims to improve quality of life for people with dementia, reduce strain on their unpaid caregivers, and enable people with dementia to remain in their homes and communities. These goals are achievable through Medicare payments for a comprehensive package of care coordination and care management, caregiver education and support, and respite services.
Brian Potvin
Brian Potvin, Director of Community Dementia Services at Episcopal SeniorLife Communities (ESLC), leads the GUIDE program and neighborhood programs that support the health and well-being of older adults affected by dementia. He also has worked for the Alzheimer’s Association and the Arc of Monroe.
Teresa Galbier
Teresa Galbier, MPA, Vice President of Dementia Program Development at ESLC, leads the creation and implementation of dementia programs in the Greater Rochester area. Previously she served as President/CEO of the Alzheimer’s Association, Rochester & Finger Lakes Region. Teresa is a member of the NYS Governor’s Council on Alzheimer’s Disease and Related Disorders, University of Rochester Medical Center Roybal Center for Social Ties & Aging Research Advisory Board as well as the SAGE II Commission of Common Ground Health.
